CARE MANAGER-STRATFORD VILLAGE
Position
Summary: The purpose of this position is to care manage patients/residents, collaborating with other disciplines and services. Such collaboration must be in accordance with current federal, state and local standards and guidelines to ensure that the highest degree of quality is maintained at all times.
Qualifications:
Knowledge of nursing technique as normally acquired through completion of an Associate's Degree in Nursing culminating in RN Licensure in the State of Michigan.
MDS certification within one year and maintain certification every two years.
Minimum of two years of experience in a long term care facility or acute care, preferred.
Must possess leadership and communication skills; creativity, integrity and initiative.
BLS certification required.
Read, write, speak and understand the English language.
Ability to make independent decisions when circumstances demand.
Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and general public.
Willing to seek out new methods and principles and incorporate them into existing practice.

The check will bounce and the victim is left responsible.
